These are the terms of service for Premium Pilot, a workers' comp premium reporting service.
What we do
Premium Pilot is a pay-as-you-go workers' compensation premium reporting service. Each payroll, it calculates the premium from actual payroll by class code, reports it to the employer's workers' comp carrier, and collects and remits the payment, for a flat $10 per payroll run. It connects to the employer's payroll provider and carrier and pulls each payroll report itself, so the employer sends nothing each pay period. Premium Pilot is not an insurance carrier, agency or payroll provider.
Fees
- $10 per payroll run.
- $0 setup.
- $0 when a pay period has no payroll.
- $40 per returned debit.
- ACH bank debit, no card surcharge.
- One payroll run = one pay date for one policy. Off-cycle and multi-policy handling: confirmed during setup.
Pricing current as of October 2026. Premium Pilot's fee is separate from your insurance premium, which your carrier sets.
Our promises
- No contract. Cancel any time. Your carrier still needs payroll reports, so we tell you the date your last report goes out and help you hand over to your carrier's portal or another service.
- Our fee is waived on your first payroll run. Your premium is still due to your carrier.
- Your fee stays at $10 per payroll run for 24 months.
Your responsibilities
Your premium depends on payroll information that must be accurate and on time. Late or missing reports can lead to estimated charges or cancellation by your carrier.
How money moves
Each payroll, we debit your bank account by ACH for your premium and our fee, then remit the premium to your carrier. Your carrier, not Premium Pilot, sets your premium and posts your payments.
You sign one authorization form during setup, and we debit your account only as that form allows.
What we are not
Premium Pilot is not an insurance carrier, agency or broker, and is not a payroll provider. We do not sell insurance, and we do not change your policy, coverage, rates or carrier.
Availability
Premium Pilot is not available in North Dakota, Ohio, Washington, Wyoming, Puerto Rico or the US Virgin Islands. Workers' comp there comes from a state fund, not private carriers. Each pay period, we get your payroll report from your payroll provider, so your payroll must track wages by class code.
Sending a request through this website does not by itself create a contract.

Some parts of this website compare Premium Pilot's fees with a competitor's. Other companies' prices and terms can change and can differ by carrier or program, so check with them before you decide. If you think something we say about another company is wrong, tell us and we will review it.

Limits on our responsibility
To the extent the law allows, Premium Pilot is not responsible for losses that come from using this website or relying on it. That includes indirect or consequential losses, such as lost profit or lost business. Nothing in these terms limits any responsibility that the law does not allow us to limit.
Changes to these terms
We may change these terms. When we do, we will post the new version here and change the date below. If you keep using the website or the service after that, you accept the new terms.
A change will not raise your fee during your 24-month price lock, and will not take away a promise above from anyone who has already started setup.
