Privacy policy
We collect what you enter in the Get Started form: details of your business, your workers' comp policy, your payroll provider, the bank account you authorize us to debit, and your signature. We use this only to set up and run your Premium Pilot service. This site sets no cookies and runs no third-party scripts or analytics.
Who we are
Premium Pilot is a workers' comp premium reporting service, and this is its website. We are not an insurance carrier, agency, broker or payroll provider.
What we collect
What you give us
If you use the Get Started form, we collect:
- whether you are a policyholder or business owner, or an insurance agent or carrier
- the authorized signer's name and title
- your company name, FEIN and business address (street, city, state and ZIP)
- your company contact's name, email address and phone number
- your insurance agent or broker's name and email address
- your carrier, policy number and policy effective date, and a copy of your policy if you upload one
- your payroll provider, your payroll client ID and how often you run payroll
- the name on your business bank account, the bank's name, and the routing and account numbers
- your signature (drawn, typed or uploaded) and the date you signed
- the authorizations you give by ticking the boxes on the form
The form also has a hidden spam-check field. People never see it or fill it in, but software that fills in every field does. If that field has anything in it, we treat the submission as spam.
What happens in your browser and at our host
Hosting logs. The company that hosts this website may keep standard server logs. These usually include your IP address, your browser type (called the user agent), the page you asked for and the time. Almost every website host does this, and the host controls those logs under its own policies. We do not use them to build profiles of visitors or for advertising.
Cookies, trackers and analytics
Our pages do not set cookies. This website does not load scripts or trackers from other companies, and it does not use analytics or advertising tools.
Why we use your information
We use what you send through the form only to set up and run your service. That means we use it to:
- reply to you by email or phone
- connect to your payroll provider and get your payroll reports
- calculate your premium and report it to your carrier
- debit your bank account and remit payments, as you authorized
- work with your insurance agent or broker and your carrier on your policy
- keep records of your agreement with us
We also use the spam-check field to throw away spam. We do not use your information for advertising, and we do not sell it.

Who handles your information
Our form-handling provider. When you send the form, it goes to our form-handling provider. That company receives your submission and passes it on to us, and it may keep a copy. It handles your information on our behalf. Like any web service, it can see technical details your browser sends, such as your IP address.
Our hosting provider. It runs this website and may keep the logs described above.
Your carrier, agent, payroll provider and payment processors. To provide the service, we share what is needed with the carrier, the insurance agent or broker and the payroll provider you name, and with the payment processors that move your payments.
Everyone else. We do not sell your personal information, and we do not share it for advertising. We may share it if the law requires it. We may also share it to protect people's safety or our rights, or if our business is sold or merged.
How long we keep it
We keep your information as long as we need it to handle your request and to keep our records. If you want it deleted, ask us. We will delete it unless we are required to keep it.
Your choices and rights
You can ask us to:
- tell you what information we have about you
- correct it if it is wrong
- delete it
- stop contacting you
To ask, use the contact details at the end of this page. We may need to confirm who you are first. That way, we do not give your information to the wrong person.
Some US state privacy laws give you more rights. For example, you may have the right to know what a business collects about you, or to opt out of the sale or sharing of your personal information. We do not sell personal information or share it for advertising. If you live in one of those states and want to use a right, contact us the same way.
Children
This website is for business owners and the people who work with them. It is not directed to children, and we do not knowingly collect information from children. If you think a child has sent us information, tell us and we will delete it.
Security
We use reasonable measures to protect your information, No website and no way of sending data is completely secure, so we cannot promise that your information will never be accessed without permission.
The form asks for a bank routing number and account number. Do not enter a Social Security number, a card number or an online-banking password anywhere on this site. We do not ask for them.

If we change how we handle information, we will post the new version on this page and change the date at the top. This version was last updated on 5 October 2026.
